In addition to providing a full-service pharmacy experience, we will offer traditional medicine access. We are partnered with Lu’ma Medical Centre that provides safe, culturally integrated primary care with a team of Elders, Physicians, Nurse Practitioners, Registered Nurses, Mental Health Counsellors, and Social Navigators. The pharmacy is integrated as part of the circle of care that braids the values of traditional and western health care, to meet the diverse needs of patients. Job Description:
The Licensed Practical Nurse will be primarily responsible for ensuring our patients receive their medications safely in the comfort of their own homes in Indigenous housing buildings. You will work within your scope of practice to provide excellent nursing care for our patients. Duties include medication administration, vitals assessment, charting and documenting of medication adherences and any relevant healthcare changes. You will act as a central liaison for stakeholders such as Physicians, Nurses, Patients and Clinics.
Job Duties & Responsibilities:
In the Indigenous Pharmacy LPN role you will:
· Deliver medications to community members living in a variety of housing, including Indigenous Housing buildings;
· Maintain proper drug storage, cold chain management, and security during delivery;
· Witness and dispense medications daily, including Opioid Agonist Treatments;
· Provide direct nursing care with a focus on administration of medications (oral meds, creams, patches, subcutaneous or intramuscular injections) as per provincial regulations, standards of practice and within your professional scope of practice;
· Maintain Medication Administration Records (MAR) and any documentation of care/services provided during visits to patients;
·Check and record vital signs such as but not limited to blood pressure, pulse, temperature and weight;
· Observe and chart changes in patient behavior and reactions to treatment and medication;
· Document the assessment and intervention of behavioral changes in patients;
· Liaise with patient supports including Tenant Support Workers at housing sites, as needed to ensure smooth delivery of medications to patients/residents;
· Communicate with patients, prescribers, healthcare professionals, and other key players to support patients through their treatment journey;
·Communicate with the Pharmacist or Pharmacy Assistant regarding any concerns from the patients about their medications, and/or insurance coverages and benefits;
· Gather and document information from patient/caregivers/prescribers in a concise, organized, and professional manner;
· Complete, submit and follow up on requisite paperwork in a timely manner;
· Inform appropriate healthcare team/healthcare professional(s) of any significant changes in the condition of any patient receiving pharmacy services from Indigicare Medicines;
· Maintain confidentiality of patient healthcare information; and
· Act as a central liaison for stakeholders such as physicians, nurses, patients and clinics and provide required updates.
Job Requirements:
· Physically and mentally able to carry out the assigned duties in the working environment;
· Passion for serving urban Indigenous community members;
· Ability to work independently with minimal supervision;
· Make decisions using sound judgment, and meeting deadlines;
· Up to date on all required vaccinations;
· Accurate data collection and data integrity;
· Ease with working in a paperless environment;
· Ability to introduce a service & share information in a culturally safe and friendly way;
· Ability to determine & achieve results and outcomes;
· Proven ability to work effectively and collaboratively in a team environment;
· Strong sense of organization and attention to detail;
· Time management skills and the ability to multi-task and adhere to deadlines.
Education, Skills and Qualifications:
· Bachelor's Degree (preferred);
· LPN certification required;
· Active practicing licensure with the British Columbia of Nurses and Midwives (BCCNM);
· Valid WorkSafe BC Standard First Aid Level (or equivalent) certification preferred;
· Valid Class 5 or higher BC Driver’s License;
· Use of a reliable personal vehicle with business class insurance and 3 million dollars liability for work purposes;
· At least 1 year of working experience in an Indigenous agency and/or serving an Indigenous community in BC;
· Knowledge of Cultural Safety & Trauma Informed Care practices;
· Excellent customer services skills;
· Experience working with Patient Support Programs (Asset);
· Excellent verbal and written English and communication skills and interpersonal skills;
· Position is subject to satisfying a Vulnerable Sector Criminal Record Clearance.
Hours and Remuneration:
Schedule: Permanent Part-Time position, Monday, Wednesday, Friday; 12 - 8pm with a .5-hour unpaid meal break.
Wage: $38 per hour
Benefits: 3 Weeks Vacation, pro rated, Employer Paid Extended Health Benefits and Mileage Reimbursement.
